Forms for student/parents
Homeschool Students at SLVUSD Charter School are required to record daily attendance and turn it in to their teacher of record every 20 days.
Each semester students, parents and teachers sign the Home Study Agreement.
Families are required to record their educational activities daily on a calendar, or attendance log. The Weekly Progress Report is a downloadable form that may be used to record daily educational activities.
Monthly Attendance Ledgers are used to summarize daily educational activities. They are required to be completed and turned into the student’s teacher of record every 20 days.
Please turn in all of the following items to your teacher of record:
- Weekly Progress Reports (or calendar pages) for each attendance week
- (4) Work Samples
- (1) Monthly Attendance Ledger
